How to deal with a disruptive student - 2

I teach fourth grade and have a similar situation with a student who always tries to get attention negatively. Usually it is because that is the only way they get attention a home. What I did is create a behavior plan. Each part of the day is broken into segments like reading, math, specials, lunch, etc... Then each segment has three parts: 1) Keeps hands, feet, and unkind words to myself. 2) Stays in seat unless otherwise told by the teacher. 3) Stays on task with 2 reminders. Each time the student obeys one of the sections they get a check. If she gets 10 checks a day then she has different "rewards" like getting to read a book to the class or have 5 min. of free time (her favorite). Also, if she gets 10 checks for 4 days she gets a larger reward like having lunch with me or the vice principal. I never do toys or food as a reward. This has worked very well. I keep the chart stapled in her agenda and send it home everyday for mom to see. Then at the end of the week I file it. Every time the student does not follow one of the rules I record it on the sheet. This way I will always have it on file if she ever gets in trouble or mom complains. I know it sounds like a lot of work but it really only takes about 5 min. out of the day and has worked well. I can e-mail you the chart if you would like. Hope this helps.
