Making your own syllabus? - ESL discussion

I am going to start private tutoring and I am interested in how people decide what to teach?
Do you make your own syllabus? Based on what?
Do you follow workbooks, such as side-by side?
I have a general idea of what should be taught at each level, beginner, intermediate, advanced. But not enough to base dozens of lessons on.
Also, what workbooks are best? I used Side-by-Side in Japan but I didnt really like it. (I am teaching in Canada so stuff with Canadian content is best)
