You (probably) have a z-class *entry* permit. When you come to China you will get your expert visa and the residents Z-class visa, which is now a sticker in your passport. This sticker is the Z-class visa.

For the medical, it's rather strange you have to bring your own form!! Never heard of that before. Unless you do your medical in your country and get that *completed*, though that should be done for your entry visa. Very strange. Otherwise they should have the form in China where you get your medical.
